Referral Methods
- Online – use our Secure Online Referral Form. You will receive a completed copy of your Referral Form when finished.
- Email – Download and complete a fillable PDF Referral Form. Then email the PDF form to ivydalewellnesscenter@gmail.
com - Fax/Mail – Download and complete a fillable PDF Referral Form, then print. Fax the document to 240-348-8500.
- Call – 770-762-1053 for Referrals. We will take basic information. Please have the insurance number, date of birth, and social security number for the referred individual.
