Looking for  COVID-19 Vaccine? Follow these 5 steps!

IVYDALE WELLNESS CENTER is now vaccinating all adults ages 65 and up, health care personnel, individuals in long-term care facilities, law enforcement personnel, and fire department personnel. You do not have to be an established patient to receive the vaccine with us. If you are new to our office and would like to come for the vaccine, please follow these steps:

𝟭. Please fill out the COVID-19 Vaccine Request Form: click here
𝟮. Please fill out the PRE-REGISTRATION Form: click here
𝟯. We will schedule your APPOINTMENT.
As soon as we get all your information from the TWO forms, we will go ahead and schedule your appointment (depending on vaccine supply).
𝟰. You will receive an EMAIL CONFIRMATION.
Once your appointment is scheduled, we will send you an EMAIL MESSAGE with your name, date and time of appointment. This email message will serve as your appointment CONFIRMATION and no phone calls will be made to you anymore. This is to redirect and unload the very high phone call volume our clinic has been receiving.
